“BEFORE AND AFTER” CHALLENGE RULES

Challenge Description:

PureHealth Research, LLC “Before and After” Challenge ("Challenge") is of 3 months duration. (the "Challenge Period").

By participating in the Challenge, each Entrant unconditionally accepts and agrees to comply with and abide by these Official Rules and the decisions of PureHealth Research, LLC (“PHR”), which shall be final and binding in all respects. PHR is responsible for the collection, submission or processing of Entries and the overall administration of the Challenge. Entrants should look solely to PHR with any questions, comments or problems related to the Challenge.

The Challenge is intended to encourage the submission of “before” and “after” pictures from PHR Customers who are users of PHR products. Participation in the promotion is open to individuals who have purchased a minimum of three (3) same bottles mentioned on the challenge request and purchased from an authorized retailer.

Gifts: Entrants who have successfully complied with the Challenge Rules will receive a $500 gift certificate The Gift certificate can be used for purchases on the PureHealth Research, LLC, website via link - www.purehealthresearch.com

a) The Gift certificate is valid for one year from the date of issuance or until the balance reaches zero, whichever comes first.

b) The Gift certificate is non-transferable and cannot be redeemed for cash.

c) Products acquired through the use of Gift certificate are not eligible for returns.

1. Eligibility. The Challenge is open to all legal United States and Canadian residents 18 years of age and older. PHR and their respective parents, subsidiaries, affiliates, distributors, retailers, sales representatives, advertising and Challenge agencies and each of their respective officers, directors and employees (collectively, the "Challenge Entities"), and members of their immediate families and/or persons living in the same household as such persons, are ineligible to enter the Giveaways or win a gift. Entrants must be in compliance with all Challenge rules. Void where prohibited or restricted by law.

The Buyer of the products and Recipient of the Gift certificate shall be the same individual, and no third-party intermediaries or assignees are permitted to assume the role of the Recipient. In order to uphold the integrity of participation in the project, thorough verification of the delivery address details will be conducted prior to dispatching the Gift certificate to the entrant. Should any disparities arise, the PHR reserves the prerogative to nullify and withhold the issuance of the Gift certificate to the victor, notwithstanding the victor’s fulfillment of all competition prerequisites.

2. Submission Requirements. The submissions include “before” and “after” questions as well as questionnaires. Requirements are as follows below:

a) All submissions must be made using the PHR’s submission forms. After enrolling in the Challenge, an email will be sent to the Entrant with instructions and a questionnaire. A follow-up email will be sent 30-100 days after Entrant’s first completed submission is received by PHR’s.

b) Pictures submitted must be of sufficient quality for evaluation following PHR’s specifications.

c) Questionnaires must be fully completed and submitted along with the required media.

d) The second email containing the submission form for “after” product pictures, video review, and questionnaire must be completed within one week of receipt of PHR’s follow-up email.

e) A response to the first email containing the submission form for “before” product pictures must be completed within one week of receipt.

f) Entrants warrant that the submitted photographs and submission data are actual photographs of Entrant and submitted information is true and accurate.

3. Entitlement. All Entrants who complete the Challenge shall be entitled to a Gift Certificate.

Entrants shall be required to sign and return an affidavit of eligibility and release of liability, and a Publicity Release (collectively "the Gift Claim Documents"). No substitution or transfer of a gift is permitted except by PHR.

4. Gift Notification. Entrants will be notified via email when they have successfully completed the Challenge. There shall be no gift substitutions and the gift is non-transferable. The gift cannot be redeemed for cash. The Entrant will be solely responsible for reporting and payment of any taxes, if applicable, as well as all other costs associated with claiming the gift.

5. Participation. By submitting an Entry, Entrants grant PHR a perpetual and unrestricted license to use their names, submitted images and likenesses in advertisements and/or promotions without further compensation. The use of personal information collected in the course of the Challenge shall be governed by the terms of the PHR Privacy Policy. By participating, Entrants agree to be bound by these Official Rules and the decisions of PHR. PHR reserves the right to disqualify persons found tampering with or otherwise abusing any aspect of this Challenge as solely determined by PHR. In the event the Challenge is compromised by tampering or other causes beyond the reasonable control of PHR which corrupts or impairs the administration, security, fairness or proper operation of the Challenge, PHR reserves the right in its sole discretion to suspend, modify or terminate the Challenge. Should the Challenge be terminated prior to the stated expiration date, PHR reserves the right to award gifts based on the entries received prior to the termination date. PHR is not liable for lost, late, delayed, damaged, incomplete, or misdirected entries.

6. Privacy. All Entrants acknowledge that certain of their personally identifying information may be disclosed to third parties at least as required by law, including, without limitation, on an Entrants' list. All personal information submitted as part of the Challenge may be shared by and between the PHR and its affiliated companies, and each of these entities may use such information in accordance with their respective independent online privacy policies. 7. Limitations of Liability. By entering the Challenge, Entrants hereby agree that all disputes, claims, and causes of action arising out of or in connection with the Challenge, or any gifts awarded, shall be resolved individually without resort to any form of class action. Any claims, judgments and awards shall be limited to actual out-of-pocket costs incurred and associated with entering the Challenge, if any. Entrant hereby specifically waives the right to claim that any attorney’s fees be awarded and under no circumstances will any Entrant be permitted to obtain any award for, and Entrant hereby waives all rights to claim, any and all punitive, incidental or consequential damages of any kind and any and all rights to have damages multiplied or otherwise increased. Entrant hereby waives any statutory, common law or other legal rights to make any claims for any amounts outside of actual out-of-pocket costs in entering this Challenge and Entrant agrees to limit its out-of-pocket costs in entering this Challenge to twenty-five dollars (US $25.00).

In the event of a dispute concerning who registered to participate in the Challenge, the registration will be declared to have been made by the authorized account holder is defined as the natural person who is assigned to an email address by an internet provider, online service provider, or other organization (e.g., business, education institution, etc.) that is responsible for assigning email addresses for the domain associated with the submitted email address. A potential winner may be requested to provide proof that the potential winner is the authorized account holder of the email address. If a dispute cannot be resolved to PHR’s satisfaction, the entry will be deemed ineligible.

8. Construction. All issues and questions concerning the construction, validity, interpretation and enforceability of these official Challenge rules, or the rights and obligations of any Entrant and their affiliated organization and PHR, shall be governed by, and construed in accordance with the laws of Nevada, without giving effect to any choice of laws or conflict of laws rules or provisions that would cause the application of the laws of any jurisdiction other than Virginia. In the event that any provision herein is determined to be invalid or otherwise unenforceable, these rules shall be construed in accordance with their terms as if the invalid or unenforceable provisions were not contained therein and the provisions deemed unenforceable or invalid by a court of competent jurisdiction shall be blue penciled as narrowly as possible in order to make the provisions valid and enforceable.
